How to Get Started
2) We Match You Thoughtfully
Our intake team reviews your needs and preferences (identity, specialties, schedule, insurance/self-pay). If you already have a CCA provider in mind, note that on your form—we’ll prioritize that match when schedules align. If they’re unavailable, we’ll offer close-fit options and/or a waitlist for your first choice.
3) Paperwork & Coverage (before your first visit)
We’ll send secure, digital forms via CHARM our client portal and electronic health records system. Completing these before you arrive helps us start strong.
What we’ll need from you:
- Insurance details (front/back of card) or self-pay preference
- Payment method on file for co-pays/coinsurance/self-pay
- Consent & intake forms (including practice policies)
- Optional: Release of Information if you want us to coordinate with other providers
What CCA does on our end:
- Verify insurance eligibility and benefits (if using insurance)
- Answer any questions about coverage, fees, or forms
4) Begin Care, Together
Depending on the situation, we’ll either book your first session for you or send step-by-step instructions to request an appointment in Charm (our client portal). Either way, we’ll outline what to expect and stay available for questions.
